Short answer: Plex can't read an M3U playlist. Its Live TV & DVR feature was built for network tuners like HDHomeRun, so adding IPTV means running a small bridge — Threadfin, the maintained fork of xTeVe — that presents your playlist to Plex as if it were an HDHomeRun tuner. Plex then discovers it, maps the channels, and gives you its normal guide, with DVR recording if you have Plex Pass. Setup takes about half an hour. Any guide describing an IPTV "plugin" for Plex is out of date: Plex removed its plugin system in 2018.

Why a bridge is needed
Plex's Live TV looks for tuners that announce themselves on your network in a specific format — the way an HDHomeRun does. An M3U file is just a text list of stream URLs; here's what's inside one. Plex has no idea what to do with it.
Threadfin sits in the middle. It reads your M3U and XMLTV, then advertises itself on the network as an HDHomeRun-compatible tuner. Plex sees a tuner, not a playlist, and everything else works as normal. This is called tuner emulation, and it's the only reliable method.
Threadfin supersedes xTeVe, adding a newer interface, better channel handling and the ability to assign backup streams to a channel. xTeVe still works, but Threadfin is the one to choose for a new setup.
Setup, step by step
1. Install Threadfin on the same machine as Plex, or any always-on machine on your network. It runs as a standalone binary or in Docker, and its web interface is on port 34400.
2. Set the tuner count. This is the number of simultaneous streams — set it to match your provider's concurrent connection limit, not higher. Watching one channel while recording another uses two.
3. Add your M3U playlist in Threadfin's playlist settings.
4. Add your XMLTV guide in the XMLTV settings.
5. Filter and map channels. Large playlists should be trimmed to the channels you actually watch — Plex handles a few hundred far better than a few thousand. Map each channel to its guide entry; mismatched IDs are why channels show no programme information.
6. Add the tuner in Plex. Settings → Live TV & DVR → Set Up Plex DVR. Plex should discover Threadfin automatically as an HDHomeRun. If it doesn't, enter the machine's IP address and port manually.
7. Confirm channel mapping in Plex's wizard and let it load the guide.
Your channels now appear under Live TV in every Plex app, with a guide, and with recording if you have Plex Pass.
Common problems
Plex finds no tuner. Threadfin isn't reachable — check it's running, and that Plex and Threadfin are on the same network. Enter the address manually if discovery fails.
Channels appear but won't play. Test the stream URL in VLC first. If it fails there, the problem is the provider or credentials, not Plex. The testing sequence is here.
Guide shows nothing. Channel IDs in the playlist don't match the XMLTV file. Fix the mapping in Threadfin.
Streams drop when recording. You've exceeded your provider's connection limit. Lower the tuner count to match it.
Playback stutters. Plex may be transcoding. Check the server's CPU during playback, and see the buffering guide if the issue isn't the server.
Worth considering first
